MedTrack offers a decentralized and interoperable electronic health records platform that links patient health data with national digital identification numbers, enabling universal access for authorized healthcare providers. This innovative solution tackles the issue of fragmented health records, aiming to reduce duplicate testing, minimize diagnostic delays, and enhance the continuity of care. Currently, MedTrack operates in over 32 public and 24 private facilities in the Gomoa East District of Ghana, serving more than 16,000 users and managing over 210,000 health records, leading to improved healthcare outcomes such as shorter patient wait times and better access to digital records. Collaborating with Ghana Health Service and MTN Ghana, MedTrack aims to provide affordable digital health infrastructure to both urban and rural areas. Their long-term goals include expanding their platform to serve 1 billion Africans, deploying in 200 Ghanaian healthcare facilities by 2027, and training 1,000 healthcare workers in digital health management by 2026. MedTrack also targets a 30% reduction in mortality rates and an 80% improvement in hospitalization outcomes by 2030, alongside plans to expand into five additional African countries by 2028, focusing on urban, general, and rural populations.

1. Technical and Infrastructure Challenges MedTrack faces significant connectivity barriers in rural areas, including limited internet access, power outages, and low digital literacy among healthcare providers. 2. User Experience and Adoption Resistance Healthcare workers express skepticism in transitioning from paper-based systems, citing poor experiences with existing platforms like LHIMS that featured unintuitive interfaces and steep learning curves. 3. Data Security and Privacy Concerns Handling sensitive health data linked to national biometric IDs raise significant security concerns. 4. Interoperability and System Integration: Fragmented healthcare systems with incompatible data formats present major challenges. None of Gomoa East District’s public facilities had electronic health records prior to deployment.
Recommendations
Rather than adopting offline-first architecture, MedTrack believes in fixing internet infrastructure in underserved areas. Solutions included partnerships with leading ISPs and Jangala, a non-profit providing low-cost internet-accessible devices to underserved communities, and collaboration with MTN Ghana for zero-data-cost services. Offline capabilities and SMS/USSD technologies serve as complementary backup solutions when connectivity is temporarily unavailable. Recommendation: Invest in improving internet infrastructure in underserved areas while providing backup access channels; sustainable digital health requires addressing root connectivity issues rather than designing around them. MedTrack addresses this User Experience and Adoption Resistance through user-centered design, device-agnostic responsive interfaces optimized for smartphones and tablets, and comprehensive training programs. Recommendation: Involve frontline healthcare workers throughout development through usability testing; prioritise simplicity over feature complexity. MedTrack implemented multi-layered security through IBM Hyper Protect Services and AWS Health Equity initiative, including AES-256 and TLS 1.3 encryption, role-based access control, and compliance with HIPAA, GDPR, and ISO 27001 standards. Recommendation: Security must be foundational; partnerships with established cybersecurity providers enhance stakeholder trust and regulatory compliance. MedTrack implemented HL7 FHIR and HL7 v2 standards for interoperability with laboratory and pharmacy systems. Recommendation: Adopt international interoperability standards from inception; design for ecosystem integration rather than standalone functionality. Best Practices Identified Phased district-level implementation before national scaling allows iterative refinement. Community engagement through health screening programs builds trust and facilitates adoption. Comprehensive training ensuring healthcare workers became system champions rather than resistors. Intuitive pricing models make solutions accessible to both public and private facilities. Cloud-based architecture enables horizontal scaling across countries with biometric ID systems. Policy and Research Implications National digital ID systems should be recognized as critical health infrastructure. Policies should mandate interoperability standards for digital health solutions. Investment in rural internet infrastructure is essential for sustainable digital health transformation. Linked biometric ID-health record database creates opportunities for population health management, disease surveillance, and evidence-based policymaking while maintaining data privacy protections.
