Health Master is an early stage implementation that aims at improving chronic disease management through the integration of AI-driven genetic analysis, real-time vital sign monitoring, and personalized care services. This innovative approach offers patients customized health insights and easy access to healthcare providers, ultimately enhancing health outcomes and quality of life. The platform tackles the difficulties of managing chronic conditions by delivering personalized recommendations based on genetic profiles, facilitating effective communication between patients and healthcare professionals, and offering home consultation and testing services. Health Master's goals include delivering AI-driven, genetically informed treatment plans to 10,000 chronic disease patients by December 2025, establishing home consultation and testing in 10 districts in Rwanda by February 2026, and increasing medication adherence by 25% through reminders and improved provider communication by March 2025. The platform aims to equip 10,000 users with real-time vital sign monitoring via its app by September 2025 and to reduce booking times for home consultations and medication searches by 50% by November 2025. The health focus areas encompass communicable diseases, cancer, emergencies, non-communicable diseases (NCDs), medications, and primary health care. The target population includes adolescents, adults, the elderly, families, health professionals, individuals with disabilities, urban and rural populations, as well as pregnant women.
