Summary
The Hi Doctor project offers an AI-driven medical simulation platform designed to enhance the clinical reasoning, communication, and empathy skills of medical students and residents. By providing interactive virtual patients, it addresses the deficits in practical, competency-based training at many medical universities through realistic, multilingual case-based learning (CBL) and OSCE simulations that are accessible anytime and anywhere. Currently, Hi Doctor is expanding its reach across Central Asia, with the goal of connecting 15 medical universities in the region by 2026 and improving students' OSCE performance by 20% within six months of usage. The project aims to make digital simulation a standard component of medical education without the need for expensive physical simulation centers, thereby enhancing both the quality and equity of medical training globally. Future objectives include developing a multilingual competency map featuring 3,000 simulated cases by 2027, integrating psychometric analytics for enhanced validity and reliability by 2026, and publishing peer-reviewed studies on the effectiveness of AI-driven simulation-based education by 2027. Ultimately, Hi Doctor seeks to scale its offerings worldwide and integrate with national digital education ecosystems, focusing on health promotion and targeting health professionals and structures.

Challenges
During implementation, Hi Doctor faced both process and technical challenges. Early on, there was resistance to adopting AI-based learning among faculty and students due to limited digital readiness and skepticism toward virtual patient effectiveness. Technical issues included integration with local university systems, inconsistent internet connectivity, and the need for multilingual adaptation across Central Asian countries. Additionally, the lack of unified digital education policies and slow regulatory approval processes delayed deployment in some regions.
Recommendations
Positive Influences and Best Practices: -Strong institutional support from medical universities, the involvement of academic leaders and ministries, and the growing national interest in digital transformation helped overcome barriers. -Hi Doctor found that co-designing curricula with educators and localizing AI scenarios significantly improved engagement and learning outcomes. Our recommendations are that, to ensure scalability and sustainability, it is important to: -Strengthen policy frameworks for digital and simulation-based education; -Provide faculty development and digital literacy programs; -Encourage international research collaboration on AI-based learning; -Facilitate integration with national digital health and accreditation systems. These steps will enhance adoption, support educational innovation, and accelerate Hi Doctor’s global expansion.
