The Community Health Toolkit (CHT) is an open-source digital platform designed for effective health service delivery at the community level. It is adaptable to local and national health system requirements and supports multiple languages, offline functionality, and various devices including feature phones, smartphones, tablets, and computers. The CHT consists of five configurable components focused on messaging, task management, decision-support workflows, longitudinal profiles, and analytics to enhance community health services. The Nursing and Social Security Division (NSSD) of the Department of Health Services (DoHS), under the Ministry of Health and Population (MoHP), oversees the Community Health Nursing Program in partnership with municipalities. They are responsible for recruiting and training Community Health Nurses (CHNs) who utilize the CHT Android app for effective data collection, surveillance, service delivery, and coordination in communities. Medic Mobile Inc (Medic), the steward of the CHT, provides support in system design, configuration, and ongoing technical assistance to the government. CHNs engage in various activities such as maintaining health records, conducting health assessments, promoting healthy lifestyles, and providing health education and counseling aimed at increasing health insurance participation. The health focus areas include cancer, communicable diseases, sexual and reproductive health, emergency response, environmental health, health promotion, primary healthcare, maternal and child health, mental health, non-communicable diseases, and vaccination. The target populations served by this initiative include adolescents, adults, children, pregnant women, the elderly, families, and individuals with disabilities.
