Integrate Health (IH) and Medic Mobile have created a mobile app designed to assist Community Health Workers (CHWs) in delivering equitable and quality care in underserved areas. The app benefits 27 CHWs by aiding decision-making, facilitating follow-ups, and enabling offline data collection, while connecting home care to health facilities through a unique patient ID system. It effectively tracks patient care over time, enhances data transmission for evidence-based decision-making, and allows health workers to log services for efficient follow-ups. The technology is adaptable, with continuous updates informed by data to improve functionality and workflow. The app focuses on various health areas, including maternal and child health, communicable diseases, sexual and reproductive health, and other chronic conditions and disabilities.
