The community-based family planning system strengthens population, health, and environment initiatives by empowering community health workers and supervisors through the Balanced Counseling Strategy plus (BCS+) guidelines. Health workers use a mobile app to assess client needs and distribute family planning methods, while supervisors monitor activities and stock levels through another app integrated with Pathfinder's DHIS for automated reporting. The Model Household system allows Population Health and Environment champions to evaluate households based on 75 criteria, identify weaknesses, and collaborate with health workers on actionable improvements, tracking progress over time. Key health focus areas include Environmental health, Health promotion, Sexual and Reproductive Health, and Communicable diseases.
