A new patient-level information system has been created to track children's immunization appointments and health from birth to age five, while also managing vaccine stock usage. This system aims to resolve critical challenges including incomplete reporting, poor population targeting, and the lack of unique identifiers for infants, which hinder continuity of care and data management. It is being implemented at the primary health care level, with a focus on vaccination and immunization.
