RAFT is a telemedicine network that spans four continents, designed to support isolated healthcare professionals by offering timely access to expertise through affordable, low-bandwidth technologies. More than just a knowledge-sharing platform, RAFT has established a strong local infrastructure to ensure the network's sustainability and maintenance. Over 15 years, the organization has gained valuable insights through South–South collaboration, diverse certification models, and various approaches to development. We are happy to share lessons learned, key takeaways, and perspectives for future growth, emphasizing RAFT's role in enhancing continuous medical education and improving access to care via telehealth. The focus areas include Primary Health Care, Maternal and Child Health, Skin Diseases, Ophthalmology, and Sexual and Reproductive Health, targeting both the general population and healthcare professionals.

Recommendations
- Institutional anchoring: For the network to grow and develop in the long term, receiving support from the hospital, university, and government is crucial. - Low operational cost: Keeping the operational cost low will facilitate the maintenance of the network. A newly implemented site should be able to work on its own and be autonomous after a while. - Multiple partnership: Multiple local and international partnerships will ensure the trustworthiness, stability, and expansion of the network. - Capacity building of local experts: Local partners should be regularly trained on how to use the system. They should be able to easily transfer the acquired knowledge and adapt it to the local context. - Demonstration of impact: More than deploying new technology and connecting health professionals, an essential motivation for both the end users and the government is to demonstrate the real impact of the system on population’s health outcomes - Bottom-up approach: This approach ensured the motivation and the engagement of healthcare professionals. Today, they have created a center of expertise in e-health and telemedicine. However, hospital, university, and government support was necessary - Top-down approach: This approach facilitated the long-term sustainability of the system. / - Bottom-up and then top-down approach: This approach ensured both the buy-in of the different stakeholders as well as the growth of the network.
