The Family Health Profile Program in Lumbini Province establishes an electronic Community Health Information System (eCHIS) that enables proactive maternal, child, and family healthcare. Using the eCHIS-Lumbini application, health workers register entire households, track pregnancies, monitor child nutrition and immunization, and conduct regular digital follow-ups through automated reminders. To date, over 224,000 households and 912,000 individuals have been registered, strengthening data-driven service delivery across the province. The program is powered by the Community Health Toolkit (CHT) – a free, open-source, and scalable digital platform for last-mile health delivery. The CHT supports multiple languages, works offline, and functions across feature phones, smartphones, tablets, and computers. Its five key components–messaging, task management, decision-support workflows, person profiles, and analytics–enable effective and data-driven community health services. The Lumbini Province Health Directorate, in partnership with district health offices, provides strategic leadership, manages the budget, and supervises implementation. It has mobilized and trained hundreds of health workers and thousands of FCHVs, while also assuming ownership of core digital infrastructure. Medic Mobile Inc. (Medic), steward of the CHT, serves as the technical partner–leading system design, testing, deployment, and capacity building through a Master Training of Trainers model, ensuring sustainability and ongoing technical support.
