Summary
The Community Health Information Platform (CHiP), powered by the global digital tool MedPlat, is designed to enhance integrated community health service delivery and reporting. It includes an Android app and optical character recognition forms for community health workers, a desktop application for facility staff to monitor performance, referrals, and reporting, as well as a leadership app to track key health indicators. Ongoing integration with Health Management Information Systems (HMIS) and SmartCare Pro is also in progress. The CHiP focuses on various health areas, including injuries, vaccination/immunization, environmental health, sexual and reproductive health, maternal and child health, non-communicable diseases (NCDs), emergencies, health promotion, chronic conditions and disabilities, communicable diseases, and mental health.
