Medic Mobile, in partnership with VHW, improves connectivity for community health workers (CHWs) to the health system via a structured work plan. This initiative enables CHWs to efficiently manage and follow up with patients through a web application accessible on smartphones and computers at local clinics. CHWs receive notifications about patient appointments and conduct home visits to remind patients of their schedules, promoting attendance at health facilities. The health focus areas include vaccination/immunization, communicable diseases, chronic conditions and disabilities, as well as maternal and child health.
