Summary
The national Picture Archiving and Communication System (PACS) is designed to provide centralized, secure storage and remote viewing of radiological images and reports, enhancing patient care through the integration of the Sanarip Clinic medical information system. This development replaces outdated local archives and film-based workflows, allowing clinicians to easily receive imaging orders and access results. The system facilitates remote interpretation by radiologists, improves continuity of care, and minimizes repeat examinations caused by unavailable prior studies, while also expanding access to specialist expertise in rural and urban health facilities. By 2027, the goal is to expand PACS coverage across public health facilities, streamline workflows for electronic orders and results, maintain a centralized DICOM archive, enable secure remote interpretation, and reduce delays in accessing radiological images. Key health focus areas include communicable and non-communicable diseases, as well as primary health care, targeting both health professionals and the general population.

Challenges

A phased rollout beginning with high-volume imaging sites, use of the DICOM standard, early involvement of radiologists and facility IT teams, and integration with the clinical workflow in Sanarip Clinic have supported adoption. Centralized monitoring, role-based access, auditability, clear equipment-integration requirements, and preservation of prior studies improve scalability, patient safety, and long-term sustainability. Remote interpretation should be introduced together with agreed service-level expectations and clinical responsibility rules.

Recommendations

Implementation requires integration with heterogeneous radiology equipment, stable transfer of large DICOM studies, sufficient central storage capacity, and reliable connectivity between facilities. Other challenges include migration from local and film-based archives, cybersecurity and access-control requirements, alignment of radiology workflows across organizations, and training clinicians and radiology personnel to consistently use electronic orders, reports, and remote viewing tools.
