Pakistan Kidney and Liver Institute (PKLI) – Integrated Digital Health Platform for a Multi-Hospital Transplant Network

The project involves the development of a comprehensive digital health platform connecting a tertiary transplant hospital, a secondary hospital, and 18 satellite clinics into a unified system. It addresses fragmented workflows by enabling real-time access to patient data, standardizing clinical processes, and improving coordination across all levels of care.
The platform integrates clinical, diagnostic, and administrative functions, enhancing patient safety, reducing medical errors, and improving operational efficiency. It supports over 1,300 users across 20 PKLI facilities and enables interoperability through HL7 and FHIR standards.
The hospital achieved JCI accreditation in 2024, reflecting compliance with international quality standards supported by the digital infrastructure. The project primarily addresses non-communicable diseases (NCDs) for the general population.
The implementation faced significant change management challenges, as transitioning from paper-based and fragmented systems to a fully digital environment required substantial adaptation by clinical and administrative staff. Standardizing workflows across a multi-hospital network with varying operational practices also required intensive coordination.
A major complexity stemmed from the project scope, defined by an extensive RFP comprising 2,499 requirements. This effectively required the development of a highly customized solution tailored to the needs of Pakistan and, in particular, a specialized organ transplant hospital. Over the course of more than eight years, continuous changes and evolving requirements led to the system becoming a highly localized digital health platform adapted to national and institutional needs, including the addition of the Urdu language.
Technical challenges included integrating multiple systems (HIS, LIS, RIS, PACS, ERP) and ensuring interoperability through HL7/FHIR standards. Additional complexities involved adapting inventory management practices (from batch-based approaches to standardized digital processes) and integrating comprehensive subsystems such as queue management and kiosk solutions.
Contextual factors, including evolving regulatory requirements and the operational complexity of a high-volume transplant center, further influenced the implementation process.
Strong stakeholder engagement was a key success factor, particularly the active involvement of hospital leadership, clinical teams, and institutional partners throughout the implementation process. This ensured continuous alignment between technical development and clinical needs.
A phased and iterative implementation approach proved essential for managing complexity in a large-scale, multi-hospital environment. It enabled gradual user adaptation, structured training, and continuous system improvement over time.
Localization and flexibility played a critical role. Adapting workflows to national practices and supporting local language requirements, including Urdu, significantly improved user adoption and long-term sustainability. Over time, the solution evolved into a highly customized digital health platform tailored to Pakistan’s specific needs, which can now be leveraged for future provincial and national-level projects.
The establishment of standardized data structures and the use of interoperability standards (HL7/FHIR) enabled scalability across multiple facilities and created a strong foundation for integration with national health systems.
Additionally, the digital infrastructure contributed to achieving a major milestone: for the first time, a Sisoft-implemented hospital in Asia — Pakistan Kidney and Liver Institute (PKLI) — obtained Joint Commission International (JCI) accreditation, reflecting compliance with international quality and patient safety standards.
Finally, combining enterprise-level digital infrastructure with long-term maintenance and support models ensured operational continuity, system reliability, and sustainable digital transformation.
