A comprehensive digital health intervention aimed at managing hypertension and diabetes was successfully executed using government infrastructure across three primary facilities. Rural adults were provided with individual digital accounts featuring unique IDs during household registration, which facilitated referrals for health measurements at community clinics. Once identified, patients were directed to the NCD Corner for additional management, receiving health education and counseling. The program integrated automated SMS notifications to deliver real-time health updates to clients, ensuring adherence to national guidelines for managing these conditions. This intervention also established a structured workflow among healthcare providers and introduced a secondary referral system to District General Hospitals, marking a significant advancement in health management strategies for non-communicable diseases (NCDs) in Bangladesh.
