The Health Facility Survey Management (HFSM) System is a web application aimed at collecting data on health facilities in Sri Lanka. It provides users with different access levels to carry out designated tasks. Internal users are tasked with completing survey forms for the Ministry of Health's Health Information Unit, Central Administration Branch, and other stakeholders, including the general public. The system encompasses various health focus areas and additional topics.
