The project aims to establish an anonymous and non-punitive medical incident reporting system to overcome the limitations of current systems, which often fail to ensure anonymity and may penalize reporters, hindering the reporting process. A voluntary, technology-supported reporting system was designed and tested in six tertiary hospitals, leading to a marked increase in the number of reported incidents.
